JOB SUMMARY:

The Madrone Care Network (MCN) Visit Nurse is part of the interdisciplinary palliative team and is responsible for assisting in the care of MCN patients in collaboration with the RN Case Manager, the Clinical Director, and other care team members.

ESSENTIAL JOB FUNCTIONS:

  • Assists patients and families in coordinating appointments, transportation, medication, caregiving, durable medical equipment, and other psychosocial needs. Works in tandem with the RN Case Manager, LCSW and PCC
  • Assesses and documents patient needs
  • Integrates nursing interventions into the multidisciplinary palliative plan of care, incorporating appropriate standards of care and practice, and patient outcomes. Assists in making sure care plans are current in the patient chart and reflect patient and team goals
  • Implements safe, competent, and efficient patient care and interventions within policies, procedures, and standards, as noted on the multidisciplinary plan of care
  • Documents the nursing process to reflect a comprehensive and integrated approach to patient care
  • Evaluates the family’s responses to patient care based upon the effectiveness of nursing interventions/actions in relationship to established outcomes, and consults with the medical staff, case manager, and support staff regarding clinical variances and recommended changes to the plan of care
  • Reviews medications with patients and families, and educates as needed to maximize compliance with the medication treatment plan.  Assists in keeping medication record in patient chart up to date
  • Teaches the patient, family and/or other care provider critical knowledge and skills necessary to accomplish self-care
  • Ensures the patient, family and/or other care provider demonstrates the knowledge and skills necessary to accomplish self-care.  Follows up and evaluates that symptom management, skills, medications, and adaptive behaviors gained as a result of teaching interventions has been successful
  • Performs in-the-field phlebotomy

SECONDARY JOB FUNCTIONS:

  • Completes care coordination and chart notes within standards set by MCN
  • Maintains MCN confidentiality standards
  • Takes responsibility for and demonstrates safe work practices
  • Models the Mission and Core Values of MCN
  • Performs other tasks as assigned

ESSENTIAL EQUIPMENT, TOOLS OR MACHINERY:

  • Laptop computer or I-pad
  • Cell phone
  • Stethoscope, blood pressure cuff, thermometer and portable stand-on scale
  • Personal vehicle

WORKING CONDITIONS:

Work may be conducted from MCN’s office, a remote and electronically secure location that has stable WiFi and internet connectivity, or a patient’s residence.  Requires frequent sitting, walking, and standing.  May have some exposure to biological hazards and infectious diseases.

QUALIFICATIONS:

  • Current California Driver’s License, auto insurance and vehicle registration
  • Current and ongoing California nursing license
  • Basic computer skills with capacity to access patient records and chart appropriately.  Able to teleconference with patients and their families
  • Capable of hearing and communicating verbally with patients, caregivers, other members of the team, and community partners
  • Ability to respect all patients irrespective of their circumstance, ethnicity, race, sexual orientation, religious belief, or political affiliation
  • Ability to perform the essential functions of the job
