TITLE Community Health Worker
REPORTS TO RN Case Manager
LOCATION MCN Ukiah Office
STATUS Non-Exempt

JOB SUMMARY:
The Community Health Worker (CHW) provides essential support and care to patients
and families in the community in collaboration with the RN Case Manager and Care
Team. The CHW assists the RN and the interdisciplinary team in developing and
implementing an effective plan of care for Madrone Care Network (MCN) patients. The
CHW provides an additional layer of physical and psychological support to patients.

ESSENTIAL JOB FUNCTIONS:

  • Meets with patients in the community (including home visits) to assist with completing needed documentation, facilitate a video conference with a clinical staff member, and/or support patients’ personal needs
  • Coordinates care daily with the RN Case manager to assist as needed in the care of all patients
  • Provides informal counseling, support and follow-up, in collaboration with the Care Team
  • Helps patients with personal needs (errands, mileage reimbursement, coordination of care for non-MCN provider appointments) per care plan
  • Supports patient enrollment process, collaboratively assisting the Advance Practice Provider and/or RN Case Manager when requested
  • Facilitates advanced care planning in collaboration with the Care Team
  • Consults and collaborates with interdisciplinary team and others involved in the patient’s care
  • Communicates with patients and families via phone, video technology and home visits per care plan and facilitates video conferencing during home visits with other team members
  • Reminds patients about appointments
  • Communicates any changes in the patient’s condition or needs to the RN and Care Team in a timely manner
  • Charts all patient encounters and appropriate patient care information in the electronic health record (EMR) within 24 hours
  • Assists with vital checks at direction of RN Case Managers
  • Participates in plan of care task execution

ESSENTIAL EQUIPMENT, TOOLS OR MACHINERY:

  • The following equipment, with the exception of the employee’s personal vehicle, will be necessary to effectively use in performing the described job functions:
  • Laptop computer or I-pad
  • Cell phone
  • Facsimile machine
  • Photocopier/scanner
  • Personal vehicle.

WORKING CONDITIONS:

  • Infrequently checks vitals and performs other medical tasks that could involve exposure to blood, body fluids, or other potentially infectious materials
  • Works with Palliative Care patients wherever they reside or are receiving care, including homes, skilled nursing facilities, hospitals, and residential care for the elderly facilities
  • Provides own transportation
  • Successfully performs the essential duties and responsibilities of the position through the use of physical activities as described below:
    • Regularly requires sitting, standing, walking, talking, use of hands, listening, and
      observing
    • Regularly requires travel, sometimes to remote areas
    • Regularly requires working at a computer for 2-4 hours at a time
    • Regularly requires use of telephone
    • Requires use of video-conferencing technology
    • Occasionally requires reaching, stooping, bending, kneeling, and lifting items
      weighing 25 pounds or less
    • Assess the patient’s safety in the home and notify RN or clinical team if patient
      requires immediate physical or medical care.

QUALIFICATIONS

  • Two years’ experience as a caregiver, Certified Nursing Assistant, or Home Health Aide.
  • Current California driver’s license, proof of acceptable automobile insurance coverage, and reliable transportation
  • Advanced computer skills with capacity to access patient records. Able to teleconference with patients and their families
  • Capable of hearing and communicating verbally with patients, caregivers, other members of the team, and community partners
  • Capacity to work independently with minimum supervision
  • Organized with ability to prioritize and multitask
  • Ability to respect all patients and caregivers irrespective of their circumstance, ethnicity, race, sexual orientation, religious belief, or political affiliation
  • Ability to perform the essential functions of the job
